Individuals may register using the registration form, or for faster service, register online (Registration Closed).

Multiple attendees from the same institution can take advantage of a discounted registration rate. The first attendee pays the regular registration rate, the second and all additional attendees will pay the second registrant rate. Each registrant must complete a separate registration form, and all forms must be attached and submitted at the same time (Fax or mail only).

A discounted rate is also available for pharmacy residents and students.

CEOs, CFOs, and other health-system administrators may register at the ASHP member price when they register with a member pharmacist from the same organization. Include the ASHP member name and number on the registration form. Completed forms must be sent together.

Cancellations

To receive a full refund, confirmed registrations must be cancelled in writing on or before September 19, 2014 (postmark or fax date). Written cancellations postmarked or faxed September 20 through October 8 will be subject to a $50 handling fee. Refunds will be issued only for cancellations postmarked or faxed on or before October 8, 2014.

Payments to ASHP are not deductible as charitable contributions for federal income tax purposes. However, they may be deductible under other provisions of the Internal Revenue Code.
